A message from the practice to Pharmacists

We have received your request explaining that you wish to prescribe Weight Loss Medication for our patient and asking us to confirm they have no medical history that would affect your prescription.

This type of work takes valuable NHS GP time away from patients who require help. Therefore:

• Please ask your patient to provide you their medical summary from their online patient record. All patients are able to access this information via online services provided they have applied to register.
• If they decline to do this, please send us a signed consent form from the patient confirming they are happy for us to share this information. We will then send you a summary list.
• Please note that we cannot guarantee that all the information you need would be in a summary. If you require a full set of patient notes for your clinician to review, your patient can print their full record, or we can provide this as a Subject Access Request within 30 days.
• If you require this information before each prescription is issued, this needs to be provided by the patient.

Any responsibility for prescribing the medication would obviously remain with your prescriber.
